Waiver and photo consent
Release of liability and assumption of risk
This agreement is between AI Hockey ("we", "us") and the parent or legal guardian ("you") of the player named in a booking ("the player"). It applies to every skate, session, private lesson or program the player books with us.
Please read it carefully. It affects your legal rights.
Hockey has real risks
On-ice hockey training is a physical, fast-moving activity. Risks include, but are not limited to:
- falls, and collisions with other players, coaches, boards, nets or the ice
- being struck by pucks, sticks or skate blades
- cuts, sprains, strains, broken bones and dental injuries
- concussion and other head injuries
- overexertion, dehydration, and aggravation of existing medical conditions
- in rare cases, serious injury including spinal injury, paralysis, or death
- the condition of the ice, rink and facilities, which are run by others, not by us
We may also make mistakes. Our own negligence could cause or add to an injury.
Assumption of risk
You understand these risks and freely choose to have the player take part. You accept these risks on your own behalf and, to the extent the law allows, on behalf of the player.
Release and waiver of claims
To the fullest extent permitted by law, you release and waive any claims you may have against AI Hockey and its owners, coaches, staff and volunteers for any injury, loss or damage connected to the player's participation, including claims arising from our negligence.
Indemnity
To the extent permitted by law, you agree to indemnify us against claims or costs arising from the player's participation, where those claims are brought by or on behalf of you or the player.
Equipment and safety rules
- The player must wear full hockey equipment at every skate, including a CSA-certified helmet with full face protection.
- The player must follow coaches' instructions and the rink's rules.
- We may remove a player from the ice for unsafe behaviour or if we believe they are hurt.
Health and concussion
You confirm the player is physically able to take part, and you will tell us about any medical condition, allergy or injury that could affect them.
If a player shows signs of a possible concussion, they will be removed from the ice and will not return until cleared by a medical doctor.
Emergency medical treatment
If you cannot be reached in an emergency, you authorize our coaches to call emergency services and obtain first aid or medical treatment for the player. You are responsible for any related costs not covered by MSP or other insurance.
Refunds and missed skates
Spots are not refundable.
We buy the ice from the rink weeks ahead and pay for it whether or not every skater turns up, and our coaches are booked for those hours the same way. A spot that goes unused still costs us the full amount, which is why a missed skate cannot be refunded or credited, and why a block cannot be cancelled part-way through for money back.
Illness, a tournament, a change of mind: the spot stays yours for the rest of the block, but the nights you miss are gone.
General
This agreement is governed by the laws of British Columbia. If any part is found unenforceable, the rest still applies. It applies to every booking you make with us until you tell us otherwise in writing.
Photo and video consent
This is optional. It has its own tick box when you book, separate from the waiver above, and you can book without ticking it.
We sometimes take photos and video at training. We use them to give players feedback on their technique, and to share on our website, social media (such as Instagram) and promotional materials.
By agreeing, you allow us to use photos and video of the player for these purposes, without payment.
If you would rather we did not photograph your player, leave the box unticked. We will mark the player on the roster so our coaches know.
- We will not post the player's full name alongside their image without asking you separately.
- You can withdraw consent at any time by emailing us — the address is at the bottom of this page. We will stop using new images of the player and remove existing ones from our website and social media within a reasonable time. We cannot recall printed material already handed out, or copies other people have already shared.
Your personal information
We collect your contact details and the player's information to run bookings, contact you about skates, and keep the player safe. We don't sell it. We handle it according to BC's Personal Information Protection Act. For questions or requests, email us at the address at the bottom of this page.
Agreement
By ticking the boxes when booking, you confirm that you are the parent or legal guardian of the player and are 19 or older, that you have read and understood this agreement, and that you agree to it on your own behalf and on behalf of the player. Your agreement is recorded with the date and time you accepted.
